In a study conducted by the Oxford Radcliffe Hospital Charitable Funds and the ResMedUK and the the National Institute for Health Research Oxford Biomedical Research Centre, 25 adults with OSA included in the study had all been using CPAP successfully for over a year at the time of recruitment. The double-blinded, cross-over design required that CPAP be withdrawn for 14 nights during each treatment arm, during which time participants received supplemental oxygen or regular air (sham) overnight via a face mask or nasal cannula.

The primary outcome was the change in home morning blood pressure following CPAP withdrawal for 14 nights, oxygen versus air. Secondary outcomes included oxygen desaturation index (ODI), apnea hypopnea index (AHI), and subjective (Epworth sleepiness score) and objective (Oxford sleep resistance test) sleepiness.

“As expected, supplemental oxygen significantly and markedly attenuated measures of intermittent hypoxia, with a median reduction in ODI of -23.8/h (P<0.001; IQR -31.0 to -16.3), and an absolute median reduction of the percentage time with oxygen saturations <90% of -9.8% (P<0.001; IQR -16.7 to -4.3), compared to air," the researchers wrote.

"There was a small significant reduction in the heart rate rises index (a measure of autonomic arousals), with a median reduction of -3.7/h (P=0.006; IQR -9.1 to -0.8), oxygen versus air. The small reduction in median AHI of -3.6/h (P=0.98; IQR -10.2 to 10.1), oxygen versus air, was not statistically significant."

No significant differences in either subjective or objective sleepiness were seen with oxygen versus air, and neither were there any significant differences seen in change in mean overnight urinary volume or overnight urinary creatinine concentration.

Thus, while it can be said that Oxygen eliminates morning BP spike in Sleep Apnea, it is yet to be determined through additional study, if the most likely candidates for supplemental oxygen therapy - those with the most severe hypoxia, will benefit from it.
